The old PCCONS driver only seems to be used on minimal install disks and
cannot coexist with newer ones, so there's probably no practical use case for
supporting it in Xorg anymore.

The only consumer seems to be one BSD specific file, the few drivers using
the *_iopl seem to include it on their own. Thus, no need to keep it in
public headers.

DEV_MEM define isn't used by any drivers, and BSD seems to be the only
platform using /dev/pmem instead of /dev/mem - as well as the DEV_MEM
define from xf86_OSlib.h (Linux uses the symbol, but defines on its own)
Therefore, just define it where actually used and drop it from the global
xf86OSlib.h file.
Note that /dev/pmem refers to physical memory, not to be mixed up with
Linux's persistent memory subsys, which uses /dev/pmem[N] device nodes.

Commit 0d4a7ed6 put the definition of pcvt_version inside #ifdef __NetBSD__
but left one use of it outside of the ifdefs, resulting in a build failure
on FreeBSD 14.0 in the gitlab CI for xf86-input-keyboard.

The definition relies on IOPortBase, which is only ever set in
hw/xfree86/os-support/bsd/arm_video.c
This caused build failures on linux/mips with GCC 10, due to this
change (from https://gcc.gnu.org/gcc-10/changes.html#c):
"GCC now defaults to -fno-common. As a result, global variable accesses
are more efficient on various targets. In C, global variables with
multiple tentative definitions now result in linker errors. With
-fcommon such definitions are silently merged during linking."
As a result anything including compiler.h would get its own definition
of IOPortBase and the linker would error out.
Most (but not all) of these were found by using
codespell --builtin clear,rare,usage,informal,code,names
but not everything reported by that was fixed.

In commit 9db2af6f75 (xfree86: Remove xf86{Map,Unmap}VidMem) we
somehow stopped exporting xf86{Read,Write}Mmio{8,16,32}. Since the
function pointer indirection was intended to support dense vs sparse and
sparse support is now gone, we can just make the functions static inline
in compiler.h and avoid all of this.

Roundhouse kick replacing the various (sizeof(foo)/sizeof(foo[0])) with
the ARRAY_SIZE macro from dix.h when possible. A semantic patch for
coccinelle has been used first. Additionally, a few macros have been
inlined as they had only one or two users.

pciaccess does this for us, and none of our internal hooks really
remain. This does remove a cleanup pass from the BSD code, but the case
it's covering (a previous server leaving MTRRs around) can't happen
anymore.

This API sucks. Fortunately it's mostly unused at this point. geode,
sis, and xgi need minor patches to use the corresponding pciaccess code,
neomagic will (more explicitly) lose its non-PCI support, and newport
will need to be ported to /dev/mem or the platform bus or something.
This should also make it pretty clear that alpha's sparse memory support
was basically not a thing anymore, very few tears shed.

This lets us stop using the 'pointer' typedef in Xdefs.h as 'pointer'
