Since most of the extension init logic (and on/off switches for them)
is driven from miext, this seems the appropriate place for the header.

The dispatcher functions are much more complex than they're usually are
(just switch/case statement). Bring them in line with the standard scheme
used in the Xserver, so further steps become easier.
It's also much cleaner to use the defines from proto headers instead of
raw numbers.

Several places outside Xi (eg. dix, security hooks, ...) need to know the
actual XI base opcode. This formerly had been done by a global variable,
which is filled on XI init. This has some drawbacks, eg. requires that
XI really is initialized before anybody else attempting to access this
variable - changes in extension init order could be dangerous.
Since extension opcodes are now (compile-time) fixed for all known
extensions (including XI), this isn't needed anymore. We can really
rely on the XI extension always having the same opcode base. So we
can drop that variable entirely and use the corresponding define instead.

The symbol controls whether to include dix-config.h, and it's always set,
thus we don't need it (and dozens of ifdef's) anymore.
This commit only removes them from our own source files, where we can
guarantee that dix-config.h is present - leaving the (potentially exported)
headers untouched.

They were defined as empty macros on all platforms except for the
long unsupported Cray systems which needed to use bitfields to define
types smaller than 64-bits.

This touches everything that ends up in the Xorg binary; the big missing
part is GLX since that's all generated code. Cuts about 14k from the
binary on amd64.

Update currentTime in dispatch loop
Unfortunately this is equivalent to calling GetTimeInMillis() once per
request. In the absolute best case (as on Linux) you're only hitting the
vDSO; on other platforms that's a syscall. Either way it puts a pretty
hard ceiling on request throughput.
Instead, push the call down to the requests that need it; basically,
grab processing and event generation.

Fallout from fecc7eb1cf, and reverts most of the
rest of that patch.
The device name is allocated and may even change during PreInit. The const
warnings came from the test codes, the correct fix here is to fix the test
code.
